Recall Announced After Bacteria Found in Vick's Nasal Spray

Procter & Gamble is recalling Vicks Sinex nasal spray in the United States, Britain and Germany after finding it contained bacteria, the company said.

Procter & Gamble said it announced the voluntary recall after finding the bacteria in a small amount of product made at a plant in Germany.

There have been no reports of illness from the bacteria, but it could cause serious infections for people with weakened immune systems or those with chronic lung conditions such as cystic fibrosis, Procter & Gamble said late Thursday.

The bacteria poses little risk to healthy people, the company said.

Cincinnati, Ohio-based Procter & Gamble said it detected the problem during routine quality control at the plant. Analysis so far shows the problem is limited to a single batch of raw material mixture involving three lots of product, which were sold only in the three countries affected by the recall, the company said.

In the United States, the recalled product is Vicks Sinex Vapospray 12-hour Decongestant Ultra Fine Mist with lot number 9239028831.

In Britain, the company is recalling Vicks Sinex Micromist Aqueous Nasal Spray with lot number 9224028832.

In Germany, the recalled product is Wick Sinex Schnupfenspray Dosiersystem with lot number 9224028833.

All recalled products are in the 15-milliliter size.

Lot numbers are listed on the outer carton and the bottle, the company said.

Consumers with the product should discard it, and they may call the company for a replacement coupon or refund, the company said. More information is at the company's Web site, www.pg.com.
Originally posted at InjuryBoard by Rick Patterson

Cardiovascular Systems ViperSheath Sheath Introducer Recalled Due to Fracturing

Cardiovascular Systems, Inc. and FDA notified healthcare professionals of a nationwide recall of all lots of the ViperSheath Sheath Introducer on behalf of Thomas Medical due to reports about stretching or fracture of the sheath during use.

In the event of a device fracture, separated segments of the device may require unplanned open surgery to remove the retained segments or control bleeding. Since this device is coil reinforced, any separation of the cannula (a flexible tube inserted into the body) has the potential to expose portions of the coil, creating the potential for vessel dissection or perforation.

The recall encompasses products distributed from March 25, 2009 to October 21, 2009, and includes the following lot ranges and catalog numbers:

Lot range: S28117 through S29174

Catalog numbers: VPR-ISH 5 X 85, VPR-ISH 6 X 85, VPR-ISH 7 X 85, VPR-ISH 5 X 45, VPR-ISH 6 X 45, VPR-ISH 7 X 45

Night Driving & Fatal Car Accidents

More than 4 in 10 fatal car accidents occur at night. When you consider the fact that there is 60 percent less traffic at night than during the daytime, that number becomes even more alarming. It is believed that the reason for this is reduced visual acuity due to the illumination from headlights.

Research is currently being conducted in Europe. Researchers are working on a project known as DRIVSCO. They have developed an electronic system that would improve driving ability at night. The work is being done on real-time vision. A microchip has been developed, which would be installed in a given vehicle making it easier to extract information from cameras such as bends or in the road. According to researcher Eduardo Ros Vidal "the aim of this chip is to support the illumination of the car, which is insufficient for ideal vision." Here's to advanced driving.

Additional information pertaining to automobile and motorcycle accidents

Originally posted at InjuryBoard by Rick Patterson

Digitek Tablets Recalled

All lots of Digitek tablets that have been sold under the labels Sold Under the Labels "Bertek" and "UDL" have been recalled due to possible double dose. The recall was announced by Actavis Totowa, the maker of the defective product. In fact, they announced a recall of all generic drugs which were manufactured at its Little Falls, New Jersey plant.

Digitek is a drug that is used treat heart failure and abnormal heart rhythms. According to the Food & Drug Administration (FDA), Actavis Totowa did not meet the required standards for good manufacturing practices. In August 2006, the FDA issued a warning letter to Actavis for failing to provide periodic safety reports at its oral dose manufacturing plant in Little Falls, N.J." And as recently as April, Actavis Totowa recalled Digitek tablets due to some of the medication containing twice the active ingredient normally found in Digitek. According to the FDA, the type of defect in this product could cause serious and even fatal reactions in users.

Should you be a consumer of this particular provider, you would be well advised to consult with your medical provider.

Women and Minimum Wage

Minimum wage is a hot topic. Why wouldn't it be. While the middle class is shrinking, the gap between the - almost all male - CEOs and minimum wage workers - largely women - has grown enormously. I believe we can surmise that can't be good for the country and/or the economy.

It is true that business owned by woman are growing at twice the national average. With that said, women business leaders have been ignored almost completely in the ongoing public debates regarding minimum wage.

If people are expected to be able to support themselves and their families without assistance, they must make a wage that makes that possible. I have to agree with Amy Lyman, co-founder and chair of the board of directors of the Great Place to Work Institute, a global research and management consultancy, when she said "Paying people a fair wage is a sign of respect and acknowledgment of the value of people's contributions to the business."When people are treated fairly and with respect, they will provide unparalleled levels of support and commitment inside the business, and to clients and customers. Everyone is more successful when people are paid a living wage."

Originally posted at InjuryBoard by Rick Patterson

Two Fatal Accidents in Santa Clarita Over Weekend

According to California Highway Patrol, two men were killed in two separate car crashes in the Santa Clarita Valley late Friday and early Saturday.

On Saturday at 3:50 a.m., 55-year-old Robert Doyle, of Saugus, allegedly fell asleep at the wheel and veered his 2003 Ford into the northbound lane of Bouquet Canyon Road just north of Shadow Valley Lane.

Doyle’s Ford collided with another Ford killing the driver, Douglas Albert Rawson, also of Saugus, according to Lt. Brian Elias of Los Angeles County Coroner’s Department. Rawson was not wearing a seatbelt.

No arrest was made and an investigation was ongoing, said CHP Sgt. Rob Lund.

On Friday evening, two hikers discovered an upside down van near Lake Hughes Road and flagged down a passing driver, who called 911, Highway Patrol officials said.

L.A. County Fire rescue units responded and found the body of Francisco Diaz, 63, of South Gate, who had been partially ejected from his car. Diaz was not wearing a seatbelt, the CHP reported.

Diaz had been traveling westbound on Lake Hughes Road east of Ridge Route Road about 11 a.m. Friday when he lost control of his Chevrolet Astro minivan and drove off the road’s edge, according to a CHP news release.

Diaz was on the job at the time delivering payroll for Los Angeles County, said CHP Sgt. Rob Lund.

The cause of the crash was under investigation Saturday.

Recall on Pediatric Tracheostomy Tubes

According to the FDA, a class 1 recall is being issued on pediatric tracheostomy tubes manufactured from July 7, 2008, through December 9, 2008, and distributed from July 24, 2008, through December 23, 2008. The tubes are manufactured by Covidien Inc.

"Class 1 recalls are the most serious type of recall and involve situations in which there is a reasonable probability that use of these products will cause serious injury or death," the FDA notes in the recall notice.

The tubes recalled are said to be difficult to assemble as they require that one component be inserted into another. One component is used to place the tracheostomy tube into the windpipe and the other is a catheter suction tube.

The company issued an urgent alert letter to customers on January 14, 2009; a second letter issued on March 6, 2009, recommends that healthcare professionals remove all remaining affected products and return them to the manufacturer for replacement or credit. The manufacturer may be contacted by telephone at 1-800-635-5267.

Specific lot numbers affected are as follows: 0806000266, 0806000858, 0806000859, 0806001328, 0807000555, 0807001444, 0807002502, 0808000387, 0808000431, 0808002285, 0810000288, 0810000289, 0810000864, 0810000865, 0810001201, 0811000228, 0811000522, 0811000523, 0811000546, 0811000547, and 0811001875.

Problems associated with the use of these products should be communicated to the FDA's MedWatch reporting program by telephone at 1-800-FDA-1088, by fax at 1-800-FDA-0178, online at http://www.fda.gov/medwatch, or by mail to 5600 Fishers Lane, Rockville, Maryland 20852-9787.
Originally posted at InjuryBoard by Rick Patterson

Body Found in Wheel Well of Plane

In what appears to be an attempted stowaway, the crew on a United Airlines Boeing 747 was checking the aircraft after landing in San Francisco International Airport, and in doing so, they came across what appears to be a male. It appears the male had crawled inside the nose gear's housing to conceal himself while attempting to leave Shanghai.

"Officials with United Airlines say the remains were found on flight 858 from Shanghai to San Francisco. The plane landed a little before 8:00 a.m. A spokesman for the airport says the body appears to be a male, based on clothing, but the gender is not entirely clear." San Mateo County coroners' office is investigating.

For more information on this subject, please refer to our section on Airlines, Cruises, Buses, and Other Mass Transit Accidents.

Originally posted at InjuryBoard by Rick Patterson

Medical Malpractice Reaches University Hospital

On Friday, a grand jury indicted Herman Brown, 40, on charges that range from rape to kidnaping. The assaults took place in the psychiatric unit of University Hospital in May of this year.

To date, the hospital has only acknowledged one of the four assaults that took place. The facility has been put on probation since they failed to provide patient safety. To make matters worse, Brown has prior felony convictions, which means it was illegal for the facility to employ him to begin with. It seems, no background search was conducted on Brown, and he is not the only one working at the hospital without a background check having been conducted. In spite of the medical malpractice, a spokesperson for the facility commented recently that they were proud of the care they provide. Well, that is reassuring, isn't it.

Originally posted at InjuryBoard by Rick Patterson
